It can’t be the perspective of the ghoul, because the ghoul can see the cat (panel 6), and it can’t be the humans because they can see the cat and not see the ghoul.
So who is this mysterious third party that can only see ghouls and humans, but not cats?
Panel 1 and panel 4 are basically mirrors of each other, where panel 1 is from the “human dimension” and panel 4 is from the “ghoul dimension”. Same relationship between for panels 2 and 5, as well as panels 3 and 6.

The people in the 4th panel arent supposed to be humans, theyre some sort of shadow people, and the 'ghoul’is like their shadow pet.
